In consideration of being permitted to use the facilities, equipment, and services of The Golfhouse ("Facility"), I, the undersigned, hereby acknowledge, agree, and represent the following:
1. Assumption of Risk. I understand that participation in golf simulator activities and use of the Facility involves inherent risks, including but not limited to physical injury from equipment, golf clubs, golf balls, and other participants. I voluntarily assume all such risks.
2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue The Golfhouse, its owners, officers, employees, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while using the Facility.
3. Facility Rules. I agree to abide by all posted rules and regulations of The Golfhouse. I understand that failure to comply may result in removal from the Facility without refund.
4. Property Damage. I agree to be responsible for any damage I cause to Facility equipment or property due to misuse or negligence.
5. Medical Authorization. In the event of an emergency, I authorize The Golfhouse staff to seek medical assistance on my behalf. I acknowledge that I am in good physical health and have no medical conditions that would prevent safe participation.
6. Alcohol & Underage Drinking Policy. The Golfhouse strictly prohibits the consumption of alcohol by any person under the legal drinking age of 21. I acknowledge that The Golfhouse does not serve, provide, or condone the consumption of alcohol by minors on its premises. I agree that The Golfhouse bears no responsibility or liability whatsoever for any underage drinking that may occur as a result of alcohol brought onto the premises by guests. I agree to take full personal responsibility for ensuring that any minors in my party do not consume alcohol during my visit. Violation of this policy may result in immediate removal from the Facility.
